It’s Never Too Early to Plan for Transitions

Everyone with an I/DD diagnosis needs a plan for life’s biggest transitions. Understanding the wide variety of services that are available during each of your or your family member’s transitions can feel overwhelming.
It is important to work with your Care Coordinator and any additional members of your loved ones’ care team at every age along the way. The People Inc. Admissions Team can also provide expert advice on eligibility, program descriptions and assistance for families.
Transition planning for school-age people typically begins 1-2 years prior to the completion of high school. Families will need this time to learn about programming and service options, gather the necessary paperwork, and have any needed assessments.
The People Inc. Family Support Services program is available for families and advocates who are living with a person with a developmental disability. Services include advocacy, training, education, and other supports. People Inc. can also assist with assistive devices and home modifications.
For students interested in community service or the opportunity to work, there are a multitude of programs that can introduce a person with a developmental disability in middle school and high school to vocational training and employment opportunities. Work with your Care Coordinator to understand your loved one’s goals and interests, navigate employment transition services, workplace readiness during the summer between junior and senior year of high school, and assessments before leaving high school. People Inc. currently supports more than 400 adults, as well as students in six school districts in various positions across WNY.
The best advice for parents/guardians is to seek advice and information early and continuously throughout the teen years.
