A senior case manager provides case management services to individuals residing in People Inc.´s Senior Housing and community members as needed. The case manager will assist with information and referral along with benefit eligibility, home care services, transportation, Meals on Wheels, adaptive equipment, housing options, and other needs. Age 62 or over are eligible.
