Certified Home Health Care was established for those with short-term health needs which can be cared for by visiting nurses, therapists, or home health aides. The emphasis is on acute conditions in which brief house visits are appropriate.

We provide these in-home services to individuals of all ages who reside in Erie and Niagara counties:

Intermittent Skilled Nursing Care- i.e. weekly skilled nursing assessment, post hospitalization, daily dressing change, weekly medication prefill or insulin syringe prefill, diet/medication, teaching to client/family, catheter change.

Home Health Aide Services - Hands-on care to assist with personal care, ambulation, transfers, meal preparation, home exercise programs.

Physical Therapy - Set up home exercise program and teach gait training, therapeutic exercises, post fractures, post hospitalizations to improve endurance and strength.

Occupational Therapy - Evaluate for adaptive equipment, activities of daily living (ADL) training, fine motor exercises.

Speech Therapy - Voice and language disorder treatments, dysphagia treatments.

Nutrition - Short term specialized diet consultations.

Medical Social Work - Assessment of social and emotional factors, assistance with long-range health care planning.

The above services are covered by Medicaid, Medicare, Private Pay and/or Private Insurance (i.e. Blue Cross/Blue Shield).
