Q. What is the Elmwood Health Center?
A. The Elmwood Health Center has been in operation at 2128 Elmwood Avenue, Buffalo, since 1994. We are affiliated with People Inc., Western New York's largest human service agency which has been helping families in Western New York, established in 1971. Elmwood Health Center is an authorized diagnostic and treatment center, certified by the New York State Department of Health.
Q. I like to be prepared. What should I bring on my first visit?
A. Ideally, you should bring the following information:
Summary of medical history (include past illnesses, conditions and surgeries)
List of current medications
Immunization records, especially for children
Previous medical records, if possible
Name and phone number of pharmacy
Medical release of information, signed
Legal guardian's name, address, and phone number, legal guardianship papers
Health insurance cards and/or ID numbers
Q. What if I get sick after hours?
A. After office hours, you may contact a nurse at the Elmwood Health Center for advice on an urgent medical need. Call (716) 874-4500. If necessary, a doctor will be consulted.
Q. I would like to contact someone in charge about my care. Who is in charge?
A. Your doctor or nurse practitioner is responsible for your health care. If you have a medical concern or comment, please share it with him or her. If you have a question about an appointment, bill, or your account, call our front desk staff at (716) 874-4500.
If you feel your concern is not being resolved satisfactorily, please do not hesitate to contact the Director of Clinical Services, Christina Sage, at the same number.
